Project in BaNES, Swindon and Wiltshire
A partnership project with Bath, North East Somerset, Swindon and Wiltshire Clinical Commissioning Group and Avon and Wiltshire Mental Health Partnership NHS Trust.
Response is working on behalf of Bath and North East Somerset (BaNES), Swindon and Wiltshire Clinical Commissioning Group (BSW CCG) and Avon and Wiltshire Mental Health Partnership NHS Trust (AWP) to complete a project to support the work underway locally to strengthen mental health support services. This project focuses on hospital admission avoidance for mental health and facilitating earlier hospital discharge.
The 1-year pilot project provides:
Housing and Community Support Workers
We Introduced two Housing and Community Support workers in 3 of the adult acute inpatient wards to support patients with housing issues and increased access to community services upon hospital discharge
The Mental Wealth Academy
A mental health support service for young people aged 16-25 years with mild to moderate mental health challenges. Young people can refer themselves or be referred by a parent/ carer. Referrals can be made by clicking here.
Review of Mental Health Services
A service review to understand people’s recent experiences of community mental health services and hospital admission for mental health. Feedback was gathered from approximately 200 people.
