If you’d like to apply for Supported Housing, the first step of the process is to complete the Oxfordshire Mental Health Partnership Referral Form which you can download here. If you’ve got any questions or need advice please do telephone our Referral Coordinator team on 01865 397951. We’ll be happy to help.
Once we have your application, we’ll contact you to arrange an assessment. This is a relaxed meeting where you can explain your needs and why you feel Supported Housing is right for you.
We’ll write to you soon after that meeting to let you know the outcome. If your application has been successful, we’ll place you onto our waiting list immediately. Waiting times can vary a lot and it’s important that you keep us up to date with any changes in your housing, contact details or needs while you wait. We need to be able to contact you quickly when accommodation becomes available.
