How to Make a Referral

At Response, we provide specialist mental health and housing support services across the community. If you are a professional supporting someone who may benefit from our services, you can make a referral using the form on this page.

Please note: Response does not accept self-referrals.

Before You Make a Referral

To help us process the referral quickly, please make sure you:

  • Have the individual’s consent to share their information with Response.
  • Provide as much detail as possible about their needs, current support, and any risk factors.
  • Include up-to-date contact information for both the referrer and the individual being referred.

If you are unsure whether our services are suitable, please get in touch with us before completing the form.

What Happens Next?

Once a referral is submitted:

  • Our team will review the information provided.
  • We may contact you for further details if anything is unclear.
  • The referral will then be allocated to the most appropriate service within Response.
  • We will contact the individual (and/or their referrer) to confirm next steps.

We aim to respond to referrals promptly, but response times may vary depending on demand and the nature of the referral.
